Seven Education are looking for a Family Hub Practitioner to work in Bolton. This is initially a 6-month role working on a part time basis (4 days a week). The role is paying £18.61 via an Umbrella Company
The role:
To work as part of the Family Hub workforce and contribute to the delivery of the universal Start for Life and wider Family Hub offer, delivering a range of preventative evidence-based interventions and support services to families, aligned to the Greater Manchester Early Years Delivery Model integrated pathways.
Act as a key contact for all families within the Family Hub, helping them to understand the support that is available to them and share information to support new and expectant parents/carers during their transition to parenthood.
Key responsibilities:
To have sensitive conversations and acting as a key point of contact in the Hub, promoting the universal open access Family Hub offer and connecting families to targeted evidence-based interventions (prioritising those that would benefit most). This can include Home Learning Environment (HLE) interventions and parenting support.
Support children, young people and families in line with Start for Life and Family Hub programmes at a universal and universal plus level, using evidence-based assessments and interventions to plan and evaluate care following clinical pathways. This will include working with families on a one-to-one and group basis.
Provide Early Help using a strengths-based approach to plan care with a child, young person and/or family when a multi-agency response is needed. To work in partnership with other key stakeholders to safeguard children and young people.
To observe, assess and enhance the development and behaviour of children, focusing on the principles of school readiness, Early Years Foundation Stage and knowledge of school age education and provision.
Promote positive healthy lifestyles and emotional health and wellbeing for children, young people, families, groups and communities, promoting self-care and relevant access to health care services, including the Healthy Child Programme, with the aim of reducing both Accident & Emergency attendances and avoidable admissions to hospital.
Make referrals to Family Hub and Family Hub Network services as appropriate to promote healthier lifestyles and build capacity in individuals.
Maintain accurate and up to dates records in line with Council guidance.
