Community Psychiatric Nurse

Seven Healthcare are seeking a qualified and compassionate Community Psychiatric Nurse (CPN) to cater to the psychiatric care needs of our community in Manchester. The successful candidate will work closely with community members suffering from mental health concerns, delivering personalized care and supporting the recovery process.
Responsibilities:
1. Evaluate the mental condition of patients through detailed observation, interviews, and understanding their medical history.
2. Provide individualized care, supporting patients with mental health problems in their daily activities.
3. Develop a top-tier standard of care, creating and implementing tailored care plans.
4. Work in close collaboration with physicians, therapists, and family members to ensure optimum patient progress.
5. Actively participate in multidisciplinary team meetings, contributing to treatment and intervention plans.
6. Monitor patient outcome and response to treatment, making necessary adjustments to care plans as needed.
7. Provide support and education to families to help them understand the patient’s condition, treatment plan, and progress.
8. Maintain accurate records of patient’s care, condition, progress, and concerns.
9. Assist patients with medication management and periodic evaluations.
Qualifications:
1. Registered Nurse (RN) degree with a valid NMC license.
2. Postgraduate qualification in mental health nursing is required.
3. Proven experience working in a community mental health setting.
4. Exceptional interpersonal skills with the ability to build a rapport with patients.
5. Knowledge of mental health conditions and treatments.
6. Excellent time management skills, ability to manage a case load effectively.
7. Strong verbal and written communication skills.
8. Subject to DBS check.
The ideal candidate should demonstrate empathy while being able to maintain professional boundaries. The role requires someone who has excellent critical thinking skills, problem-solving abilities and can make decisions under pressure. A valid driving license is strongly preferred as the role involves traveling around the community.
