Assertive Outreach Support Worker

Seven Resourcing are currently recruiting for an Assertive Outreach Support Worker to start a new role in Manchester.

As an Assertive Outreach Support Worker, you will be based in the Transitional Safeguarding Team, which supports young people aged 16 to 25 who are at risk of exploitation, abuse, neglect, or homelessness as they transition from children to adult services. This role involves proactive outreach to engage and build trusting relationships with young people, especially those who may be reluctant to engage with traditional support services.

This is an interim role initially until March 2025, with the possibility of extension, paying £21.08 an hour.

Responsibilities

• Conduct proactive and assertive outreach to locate, engage, and build rapport with young people who may be vulnerable or disconnected from support services.
• Use a non-judgmental and empathetic approach to develop trusting relationships with young people, respecting their autonomy while supporting them to make informed choices.
• Develop and implement citizen support plans to meet identified social needs of individuals, enabling them to achieve maximum health and well-being.
• Establish and maintain relationships with citizens and work proactively with appropriate stakeholders to assess individual needs and support positive outcomes.

Applicant Requirements

• Experience: Direct experience of working and engaging with vulnerable people to help them achieve the best possible outcomes in their lives, and an understanding of some of the challenges faced by young people transitioning from child to adult services.
• Qualifications: NVQ Level 4 in Health and Social Care.
• Compliance: Enhanced DBS and Right to Work in the UK.
• Hours: 35 hours a week.
• Travel: Full UK driving licence.
