Band 6 Mental Health Practitioner (Social Worker)

Seven Social Care is looking for a qualified Social Worker, for a vacancy within an NHS Community Mental Health Service.

The role:

We are looking for a qualified Social Worker to provide mental health interventions to vulnerable adults in the community. 

As a member of the multidisciplinary team, the post holder will work collaboratively with multiagency colleagues to ensure a carefully planned ‘safety network’ of care is available.

Key Responsibilities:

• Care Coordination
• Mental State Monitoring/Assessment
• Risk Assessment
• Medication Management
• Safeguarding
• Care Act Assessments

Applicant Requirements:

• Qualifications: Degree in Social Work + registered with Social Work England.
• Experience: Minimum of 2 years post qualifying experience in Social Work, with experience in Mental Health.
• Compliance: RTW in the UK with no restrictions.
• Hours: 37.5 hours p/w.
