Children and Family Worker

Seven Social Care has an oppurtunity avvailable for a Children and Family Support Worker in the Buckinghamshire area.
Job Purpose
To provide direct support and advice to clients, their carers and families with a range of complex and challenging needs and to support the work of the team.
Main Responsibilities and Duties
- Support/organise or provide practical support to clients
- Provide basic coaching and enablement support to clients
- Liaise with colleagues and other agencies to support clients with a range of complex needs and their families/carers.
- Monitor and report on clients’ circumstances.
- Organise and maintain records and documents using the appropriate process/ system.
- Carry out all duties with an awareness and understanding of the Safeguarding requirements within the area of responsibility.
- Identify eligible client needs and organise services to meet those needs by direct and/or third party provision of advice and support.
- Monitor, support and review need and services provided for clients, their carers/families.
- Liaise and work in partnership with BCC professional care groups and external agencies such as GPs, health agencies, emergency services as appropriate.
- Monitor and report on clients’ circumstances to the standards required.
- Organise and maintain confidential records, reports assessments and other documents using the appropriate process/ system.
- Carry out all duties with an awareness and understanding of the Safeguarding requirements within the area of responsibility.
- Conduct assessments of clients’ circumstances and issues to determine intervention.
- Plan and implement interventions and actions for allocated cases. Monitor and review cases and undertake less complex casework. Assist in more complex cases under supervision, or where appropriate shadowing experienced colleagues.
- Coordination and/or provision of interventions such as advice, guidance and direct support and therapeutic interventions to clients.
- Multi-professional liaison to gather and provide information, to support clients and co-ordinate interventions where required.
- Organise and maintain confidential records, reports assessments and other documents using the appropriate process/ system. Prepare standards reports as required.
- Research information to support services to client groups.
- Carry out all duties with an awareness and understanding of the Safeguarding requirements within the area of responsibility.
Requirements
Relevant care qualfications such as
- Relevant care qualification, such as NVQ Level 2/3 or equivalent.
- Appropriate experience of working with the relevant client group and understanding of the relevant statutory frameworks / requirements.
- ICT skills including use of Microsoft applications.
