Dynamic Qualified Practitioner

Seven Social Care is looking for a Dynamic Qualified Practitioner considering a new challenge in Portsmouth.
Opportunities currently exists in the Adult Safeguarding team, community teams and Discharge to Assess (D2A).
- The post holder will work with a wide range and variety of statutory and non-statutory agencies including the police, community health teams, residential homes, domiciliary care agencies, acute hospitals, substance misuse, homeless services, quality teams, CQC and local charities
- Posts are community based and will involve some home visits and candidates will be required to make visits in the local Portsmouth area.
The rate of pay is £37.59 Per Hour.
The role:
– Assessment of adults with care and support needs and their carer’s, identifying creative and personalised ways to provide support.
– Working with a range of individuals, often with complex and complicated needs:
-Triage and prioritising of cases and telephone enquiries.
-Participation and commitment to all aspects of adult safeguarding and
completion of S42 enquires when required.
-Continuing Care assessments.
-Care Act assessments.
-Mental Capacity Assessments.

- Independent and autonomous working and making decisions based on assessment information and outcomes and all aspects of decision making in relations to caseload management
- Implementation and understanding of The Care Act, Mental Capacity Act and Making Safeguarding Personal in all aspects of practice and decision making.

Applicant Requirements:
- Have a Degree in Social Work, and registered with Social Work England
- Experience in a similar role
- HCPC registered, enhanced DBS
- Car driver
- 37 hours per week
