Seven Social Care is working with Derbyshire County Council to recruit a Leaving Care Worker for their Leaving Care Service in Chesterfield.
£16.98 per hour.
We are looking for Leaving Care workers who values meaningful relationships with a strong corporate parenting ethos to join our Leaving Care Service, based at young persons hubs in Ilkeston.
Job Purpose:
To be the allocated case worker for children leaving care from age 16 up to age 25 facilitating a positive transition into adulthood and improving outcomes.
To identify the individual needs of allocated care leavers and provide support, guidance and information in a wide range of areas including health, disability, education, training, careers, employment, accommodation, managing finance, etc, ensuring key statutory outcomes are achieved.
Duties:
Ensure care leavers are proactively supported by undertaking regular statutory Pathway Plan assessments and using evidence based practice to review progress and inform the setting of agreed goals to achieve successful outcomes.

– Develop expert knowledge of the Local Offer to care leavers, leaving care statutory guidance, local and national practice guidance and research and be accountable to statutory Care Leaving outcomes, including accommodation, keeping in touch and participation in education, employment and training.
– Ensure high levels of knowledge and consistent application of relevant safeguarding procedures, polices, legislation and emerging themes learned from serious case reviews to identify risk and ensure safety.
– Keep accurate, detailed and up to date records and provide input/prepare reports for various multi agency, specialist and legal meetings.

Experience:
Working with children and families with complex needs.
Undertaking statutory children and families’ needs assessments and developing action plans to achieve lasting change
Engaging and maintaining relationships with a range of partners and external agencies and providing information and advice
Knowledge:
Understanding the development of children and young adults
Ability to creatively communicate and engage with vulnerable young people and establish and maintain professional boundaries.
Knowledge of and commitment to achieving key statutory outcomes for children in care and care leavers.
Thorough understanding of safeguarding policies and thresholds for children and adults together with a knowledge of key children in care legislation, practice guidance and the local care leavers offer
Ability to form relationships with all family members, carers and key contacts and to challenge in order to help families change.
