Locality Community Social Worker

Seven Social Care has several qualifed Social Worker roles available in the beautiful picturesque Yeovil area.

The role:

• Undertake assessments of risk and care needs in relation to service users and carers so that, within relevant legislation, appropriate action is taken, monitored, and reviewed effectively
• Make ongoing assessments of allocated cases to reflect individual circumstances including assessment of risk, enabling independence and need to ensure that service users are protected from significant harm

Key Responsibilities:

• Undertake initial visits
• Initiate and carry out local recruitment activities and analyse the effectiveness of methods used.
• Carry out stringent assessment and completion of Form F to standard, presenting reports to panel.

Applicant Requirements:

• Qualifications: Degree approved by the Health and Care Professions Council (HCPC).
• Experience: 2+ years working with disabilities as a qualified Social Worker in the UK.
• Compliance: Up to date HCPC registration and DBS.
• Travel: Clean UK driving license with the ability to commute to and from work independently.
• Hours: 37.5 hours p/w.
