Qsw – Adults – Transitions

Seven Social Care are currently seeking a dedicated and passionate Qualified Social Worker (QSW) for Adults – Transitions in Merton. This role will base around assisting young adults with disabilities as they transition into adult social care services.
Responsibilities:
1. Implement personal transition plans to assist young adults with disabilities during their transition from child services to adult care services.
2. Conduct assessments, including but not limited to; Care Act, Mental Capacity, and Best Interest, in relation to young adults’ needs.
3. Facilitate access to appropriate health and social care services, ensuring empowering and person-centred support.
4. Collaborate with other agencies such as education and health to ensure holistic care plans for service users.
5. Manage complex caseloads effectively, and maintain accurate and up-to-date records.
6. Address and mitigate any risks identified, implementing safeguarding protocols where necessary.
7. Provide advice and guidance to service users and their families, helping them to make informed decisions about the services they receive.
Requirements:
1. Must hold a valid Social Work England registration.
2. Degree or equivalent in Social Work.
3. Established knowledge of the legislative framework pertaining to adult social care, and children transition to adult social services.
4. Prior experience working in a similar role, preferably with transitioning young adults.
5. Experience in carrying and managing complex caseloads.
6. Excellent communication skills, with the ability to engage effectively with a variety of stakeholders.
7. Proven ability to conduct detailed holistic assessments and devise suitable plans.
8. A driving license and access to a vehicle may be advantageous.
