Seven Social Care is looking for a qualified Social Worker to fill an exclusive opportunity in Doncaster.
The role:
An exciting opportunity has arisen for a full time Social Worker to join the Integrated Support and Assessment Team based in the Civic Office. The Integrated Support and Assessment Team are often the first point of contact local residents or professionals have with Adult Social Care and our ethos is to offer support and guidance to residents of Doncaster and their families. We strive to maintain a person centred approach that enables individuals to identify their own strengths, what is important to them, what support they may require and how to maintain their independence wherever possible, while working closely with both internal and external partners. Each intervention is short term work, often unplanned requiring a 72 hour response to ensure that safety and wellbeing of the person is being maintained before transferred to the locality team for longer term support and assessment.
The Integrated Support and Assessment Team is currently working a hybrid model with staff members working in the Civic Office two or three days per week depending on the needs of the service. You will be expected to complete face to face assessment visits either in a person home or suitable environment, and if required workers will undertake formal assessment of needs in line with the Care Act, and provide proportionate support, ensure the person is safe and well.
Key Responsibilities:
??? You will receive professional guidance and supervision from the Team leader, Advanced Social Work Practitioners managing the team. There is an expectation to attend monthly team meetings, and 1-1 supervision sessions.
You are required to have relevant experience of assessment using the Care Act 2014, Mental Capacity Act 2005, GDPR and a good knowledge of safeguarding. Further essential skills and knowledge in recording and report writing, strength based approach to assessment with an aim of positive outcomes, awareness and experience of risk assessment are key to the role. You will also work with carers to support, signpost and assess their needs in accordance with the Care Act 2014.
- Carrying a caseload, undertaking holistic, strengths based assessments with adults, carers and families to help them make informed choices and decisions, enabling them to clarify and express their needs
Undertaking person centred strengths based care act assessment to identify need taking the appropriate action whilst respecting individual values, cultures and diversity.
During a strengths based assessment of need will assess and provide minor pieces of equipment or adaptations to support independence and reduce reliance on formal care to promote independence, prevent admission into hospital/residential care in order to manage transfers of care in a holistic, person centred way
Providing written and verbal records and reports which are concise, factual and informative based on analysis of evidence which can be used within the legal context and where necessary give evidence in court in relation to care or other proceedings. (with support, guidance, joint work where necessary on complex cases)
Applicant Requirements:
- Qualifications: Degree approved by the Health and Care Professions Council (HCPC).
- Experience: 2+ years working with children as a qualified Social Worker in the UK.
- Compliance: Up to date HCPC registration and DBS.
- Travel: Clean UK driving license with the ability to commute to and from work independently.
- Hours: 37.5 hours p/w.
