Social Worker – Assessment Team

Seven Social Care are in search of a committed and compassionate Social Worker for our Assessment Team based in Exeter. In this role, you will be tasked with ensuring the safety and wellbeing of service users and providing high-level professional advice to colleagues and partner organizations.
Primary Responsibilities:
1. Handling initial assessments and regular reassessments of individuals and families who are or may be in need of social care services.
2. Developing, coordinating, and delivering effective care plans based on these assessments.
3. Engaging with service users and their families to gauge their needs and goals, ensuring their opinions are integrated into care plans.
4. Liaising and collaborating with other professionals and agencies as required.
5. Managing your caseload while maintaining high-quality standards and meeting specified timescales.
6. Participating in multidisciplinary meetings and case conferences.
7. Maintaining thorough and up-to-date records on all cases.
8. Continuing your training and professional development to keep abreast of changes in social work practices and legislation.
Key Skills and Qualifications:
1. Qualified with a Social Work degree or equivalent (e.g., DipSW, CQSW).
2. Registered with the Health and Care Professions Council (HCPC).
3. Substantial post-qualification experience within a similar assessment-based role.
4. Knowledge of current legislation relevant to social care.
5. Strong interpersonal and communication skills.
6. Ability to work under pressure and prioritise effectively.
7. Proficient in standard IT software.
Working hours: Full time, with potential need for occasional weekend or evening work.
