Social Worker- Assessment Team

Seven Resourcing are looking to fill a Social Worker – Assessment Team role in Chatham, Kent.
This is a temporary role and is offering up to £40 ph
Key Responsibilities:
Conduct Assessments: Perform in-depth assessments of individuals and families, utilizing standardized assessment tools, interviews, observations, and collateral information from relevant parties. Evaluate social, emotional, psychological, and environmental factors to identify needs, risks, and strengths.
Gather Information: Engage with clients, families, and other professionals to collect relevant information about the client’s history, living conditions, social networks, and any significant events or issues. Obtain and review relevant documents and records to inform the assessment process.
Identify Needs and Risks: Analyze assessment findings to determine the specific needs and risks faced by clients. Identify factors that may impact the individual or family’s well-being, such as domestic violence, substance abuse, mental health issues, child protection concerns, or inadequate support systems.
Develop Intervention Plans: Collaborate with clients, families, and relevant stakeholders to develop comprehensive intervention plans based on the assessment outcomes. Ensure that interventions are tailored to address identified needs, promote safety, and enhance the client’s functioning and self-sufficiency.
Referral and Coordination: Make appropriate referrals to internal or external services, such as mental health agencies, substance abuse programs, housing services, healthcare providers, or legal resources. Coordinate with other professionals and agencies involved in the client’s care to ensure a coordinated approach.
Documentation and Reporting: Maintain accurate and up-to-date client records, documenting assessment processes, findings, and interventions. Prepare clear and concise reports summarizing assessment outcomes, recommendations, and progress toward goals.
Crisis Intervention: Respond to crisis situations involving clients and families promptly and effectively. Assess risk levels and implement appropriate crisis intervention strategies, including making emergency placements or arranging for immediate support.
Professional Development: Stay informed about relevant legislation, policies, and best practices in the field of social work. Attend training sessions, workshops, and conferences to enhance knowledge and skills related to assessment techniques and interventions.
Applicant Requirements:
- Experience: 2+ years working in a similar role.
- Compliance: Up to date DBS/SWE Registration
- Travel: Clean UK driving license with the ability to commute to and from work independently.
- Hours: 37 hours p/w.
