Social Worker – Disabilities Team

Seven Social Care are looking for a passionate and dedicated Social Worker to join our Disabilities Team based in Chippenham. The successful candidate will be empathetic, patient, and have a deep understanding of disabilities, either from personal experience or professional training.
Responsibilities:
1. Assess, plan, implement and evaluate packages of support to enable individuals to lead the fullest lives possible.
2. Liaise effectively with a range of service users and their families, maintaining good partnerships with relevant community groups, local housing teams, and health services.
3. Contribute to the safeguarding of adults by ensuring you are aware of your role in relation to the safeguarding adults policy, ensuring it is integrated into practice.
4. Maintain professional standards and continuous improvements in the quality of care and support plans.
5. Work as part of a dedicated team to provide a holistic, person centred care plan.
6. Advocate on behalf of individuals and groups to enhance their wellbeing.
7. Work flexible hours to meet service needs, that might include evenings and weekends.
Qualifications and Experience:
1. Minimum of a Bachelor’s degree in Social Work.
2. Must be registered with the Health and Care Professions Council (HCPC).
3. Previous experience in the disabilities field is required.
4. Ability to work both individually and as part of a team.
5. An understanding of the rights and needs of individuals with disabilities.
