Assessment and Enablement Support Worker – Adult Social Care Team | £12 – £16 P/H | Northamptonshire

We are looking for between 10 – 15 workers to undertake time limited work (12 weeks) within Northamptonshire Adult Social Services (NASS). The team will have a mixture of qualified social workers and unqualified assessment staff (with a majority of qualified staff). You will need to be reliable and have the relevant expertise and resources to support us with clearing a backlog of adult social care assessments and reviews.

The brief is to work with service users and their carer’s in their homes or place of residence to assess / review their care and support needs in line with the Care Act 2014.

The output from this will be:

– High quality strength based assessments and innovative care and support planning

– Best use of assistive technology, community resources and voluntary organisations

The work will involve undertaking assessments, initial reviews and scheduled reviews to address pending lists and maximise efficiency potential (savings) by appropriately adjusting support plans.

As I am sure you are aware like most local authorities we are facing the need to make significant savings at the same time as facing increasing pressures and ensuring improved outcomes for individuals.

The work required will fall into two categories:

There will be an expectation that staff (WTE) undertaking the reviews will do a minimum of 15 initial reviews or 8 scheduled reviews per week according to location and provision. Some reviews may also require a needs assessment and support plan according to time since last review.

We will provide a very clear brief to the staff and provide social work management support and supervision.

We need to ensure that going forward any savings / changes made are sustainable and more importantly are still meeting the needs of the individual.

We have large numbers on our pending list awaiting allocation. We require a small number of people to work through the pending lists, carry out any required assessment and produce the care and support plan. The expectation is that staff (WTE) will undertake 10 assessments per week
