Seven Resourcing are looking for a Community Care Worker to join the Disabled Children’s Services (DCS) Support and Advice Team (SAA) in the Referral and Assessment Team (RAT) in Exeter, Devon. This is a full time position and is paying £12.45 per hour.

The role:

Support and Advice Team are responsible for Short Break assessments and plan reviews for over 1500 children and young people with disabilities. There are 3 work streams in Support and Advice: Transition Team – supporting young people from the age of 15 to 18, the Reviewing Team – undertake reviews of Short Break plans and Referral and Assessment Team – undertake initial short break assessments and plans. We are ideally looking for CCW that has experience or an understanding of children with a range of additional needs and the complexities that this may bring. This CCW will be based in the Referral and Assessment Team. You will use a Screening Tool to assess eligibility for DCS and undertake Short Break Assessments that are needs led and strengths based. The assessment will consider if the child or young person has any unmet social care needs where these needs can be met this could range from signposting to community resources or compiling a short break plan with a provision of services, working closely with parents and carers to complete this process. You will be expected to work in partnership with our colleagues across the service including Health, Education and Social Work teams. The successful candidate will be joining a caring and motivated team that strives to improve the lives of children with additional needs and their families. Key Responsibilities:

  • Experience or understanding of young people with complex special/additional needs. –
  • Knowledge of Safeguarding practices –
  • You can work as part of a team and be able to communicate effectively with young people, parents, and professionals. –
  • A good telephone manner and ICT skills to ensure the data collected is recorded accurately and effectively, you must be able to create a comprehensive assessments and plans.
  • The ability to prioritise and work within a fast-paced team
  • The ability to work independently, whilst knowing when to seek further support
  • An awareness of SEND (Special, Educational, Needs and Disability) and EHCP (Education, Health and Care Plans)

Applicant Requirements:

  • Qualifications: NVQ Level 3 in a relevant subject that supports the posts ability to deliver change and improve outcomes for children and young people with five years’ professional experience or NVQ Level 4 with two to three years’ experience in a relevant subject.
  • Experience: 2+ years working within a similar role.
  • Compliance: Current DBS check.
  • Travel: Clean UK driving license with the ability to commute to and from work independently.
  • Hours: 37 hours p/w.
