Seven Social Care are looking for a temporary Review, Assessment Support Officer to work in Preston. This position is initially for a period of 3 months and is going to be paying £11.21 per hour (paid to you via an umbrella company).

The role:

To contribute towards an initial screening and contact and overview assessment service under the direction of the Team Manager / Social Worker and to commission services as appropriate. To contribute to the review process under the direction of the Team Manager / Social Worker.

The post holder is expected to carry out their duties and responsibilities in accordance with the County Council’s

Policies and Procedures and the Directorate’s Statement of Principles and Standards of Conduct.

To instil, promote and maintain working relationships with all levels of management, employees and service users (including other Directorates of the County Council and external agencies) which fully reflect the Directorate’s Standards of Conduct and Statement of Principles.

Key Responsibilities:

      To contribute to an initial assessment at both Contact and Overview Assessment levels as described within the Single Assessment Process under the guidance of the Team Manager / Social Worker.

To contribute to the assessment process for equipment/minor adaptations as part of the Single Assessment Process under the guidance of appropriate professionals.

To provide care plans and commission services following the completion of assessments under the direction of the Team Manager / Social Worker.

To contribute to the initial reviews of assessments and make amendments to care plans/commissioned care plans as necessary, under the guidance of the Team Manager / Social Worker.

To liaise with other partners e.g. Health, Housing as directed to ensure provision of services to meet identified needs.

Applicant Requirements:

  • Qualifications: NVQ in Health and Social Care.
  • Experience: 2+ years working within a similar role.
  • Compliance: Current DBS check.
  • Travel: Clean UK driving license with the ability to commute to and from work independently.
  • Hours: 37 hours p/w.
