Seven Social Care are looking for a couple of Social Care Assessors to work with Staffordshire County Council’s Adult Social Care Team. This role is based in Stafford and is initially for a period of 4 weeks. The hourly rate is £14.45 per hour (paid to you via an umbrella company).

The role:

Under the direction of the Case Manager, the Social Care Assessor will work closely with colleagues in both Social Care and Health, Health, District Council, Independent and Voluntary Sector ensuring that our services and those of others meet the purpose and principles of the Directorate.

Under the direction of the Case Manager the Social Care Assessor will be responsible for assisting people to get the right support in the right environment to enable them to remain as independent and socially included as they want to be.

Key Responsibilities:

      Assisting colleagues in screening initial approaches to the Directorate for assistance.

Initial assessments of need and assisting colleagues to prioritise the urgency of intervention.

A range of assessments based on a person centred approach within the context of the White Paper: Our health, our care, our say.

To accurately record needs and to ensure that these records are confidential, but readily available to team colleagues

To include and involve other professionals, and other individuals as appropriate.

Applicant Requirements:

  • Qualifications: NVQ in Health and Social Care.
  • Experience: 2+ years working within a similar role.
  • Compliance: Current DBS check.
  • Travel: Clean UK driving license with the ability to commute to and from work independently.
  • Hours: 37 hours p/w.
